We are a specialist in home care Coffs Harbour provider delivering daily living support, support coordination services and skills development programs for NDIS participants.
Our range of in home care Coffs Harbour services includes supporting NDIS participants to remain engaged with the local community and to live independently as far as possible. We do this through the provision of disability support, assistance in the completion of daily living activities, and a range of nursing services that support and promote wellbeing.
We also provide supports to assist participants in all types of daily living tasks, whether in your own home or in shared NDIS accommodation. This can include the completion of household tasks, such as cleaning or gardening, as well as the maintenance of personal hygiene and health. Memba Care trained support workers can also provide medical support for clients with complex needs to enable them to remain at home rather go into a care facility.
The Memba Care team provides group and centre-based supports for NDIS participants in addition to our in home care Coffs Harbour services.
These are designed to give clients the opportunity to take part in a range of activities in a welcoming, inclusive environment, as well as to develop social, daily living and friendship skills. This also helps people with NDIS plans to avoid feelings of social isolation, while also providing practical assistance to attend healthcare appointments.
In addition, our support workers provide personalised, community-based nursing care for participants with a range of medical conditions. This can include diabetes care, assistance with the administering of subcutaneous Injections, or the management of a stoma or catheter.
We are highly experienced plan managers and can provide NDIS support coordination in addition to our centre-based and in home care Coffs Harbour services. Our supports includes assisting clients to become NDIS participants and get a plan approved, and then helping them to find appropriate services and providers once a plan is in place.
As a further part of this support, we also work alongside participants to develop appropriate skills and capacity to understand how their NDIS Plan works, and the ways in which they can get the most out of service agreements once they in operation. Our skills development programs also assist participants to develop capacity in a range of areas that support independent living.
This can include support for daily living activities, like shopping, cooking, gardening and more, in order for participants to be better equipped to remain living at home or in shared NDIS accommodation.
Everything we do at Memba Care is designed to support NDIS participants to live as independently as possible. As part of this, our support workers help clients to find appropriate accommodation, either on a short-term or permanent basis, and then to develop the capacity to maintain their tenancy In addition, we can also provide temporary accommodation for participants in shared accommodation (for up to 28 days a year) that enables family members providing informal care to have some respite from their responsibilities.
We also provide specialist support focussed on helping NDIS participants to manage important life transitions (e.g., moving house), as well as assisting clients to find work and then develop the skills and capacity required to remain in employment.
