The aim of community-based support is to enable NDIS participants to take part in a range of local community activities and to assist you to live as independently as possible. Our range of community-based support services can take the form of either in-home care or centre-based care, and are designed to help you participate in recreational activities, take part in sporting events as a player or spectator, enjoy a wide variety of social and cultural events, and much more.

The benefits of community-based support

Community-based support is a way of helping people to avoid feelings of isolation, and to overcome some of the  barriers that may be preventing them from participating in all sorts of activities, and restricts their day-to-day living. It also enables NDIS participants of all ages to develop capacity and skills so that they are better equipped to undertake a range of daily living activities and live independently as far as possible.

Our services include supporting clients to take part in courses and other learning activities, or joining hobby and social groups. Support workers also accompany clients on excursions or visits to places of interest, as well as helping them to go shopping, visit their GP, attend medical appointments, do their banking, visit the library, and much more.

Help with daily living activities

In order to help you increase your ability to participate in the wider community, support workers also assist participants with a wide range of daily living activities (i.e., communication, self-care and mobility) as a means of helping to build capacity and confidence. 

This can include assistance with shopping and meal preparation, household tasks like cleaning and laundry, and other activities and tasks that contribute to a participant’s overall quality of life. We can also provide assistance with home maintenance tasks, such as mowing the lawn, fixing leaks, etc.

Support for personal care

Personal care also makes an important contribution to a participant’s capacity to take part in community-based activities. Our in home care Brisbane services therefore include provding a wide range of personal care to help participants maintain health and wellbeing in a variety of ways. This can include assistance with showering, dressing and toileting, as well as nursing services such as administering medication and wound dressing, or allied health services like podiatry and physiotherapy, all in your own home. 

NDIS support coordination and plan management

Our specialist NDIS support coordinators and plan managers can advise and guide you through the process of applying to become an NDIS participant, and getting a plan approved. NDIS funding pays for services under three categories:

  • Core Supports
  • Capacity Building Supports
  • Capital Supports

Once you have had a plan approved, we can help you to choose the most appropriate supports for your specific needs and circumstances, and assist you in the efficient management of your plan to ensure that the services to which you are entitled are being delivered, and that you are receiving value for money.

Other forms of funding

If you do not have a NDIS plan but still want to access community-based support or in-home care Brisbane services, there may be other forms of funding that you might be able to access, such as through the state government, via the Commonwealth Home Support Programme (CHSP), or via alternative forms of private funding.
