Memba Care provides a range of community-based nursing care services for NDIS participants, which can usually be delivered in your own home or in shared NDIS accommodation. This service is designed to provide support and assistance for people with a disability who have specific medical conditions that require a range of care levels, and can be tailored to meet your particular needs and circumstances.

As experienced in home care Brisbane providers, you can rely on our trained and experienced nursing team to provide conscientious and compassionate care at all times. Our qualified NDIS nurses come to you when it is most convenient, and work in conjunction with your doctor and other allied health professionals to provide the nursing supports you need to be able to maintain daily activities and to live as independently as possible.

Benefits of community-based nursing care

All of our community-based nursing care services are designed to meet participants’ individual needs, and are at all times focused on delivering the highest levels of health and wellbeing. NDIS nurses will work closely with your other healthcare providers to ensure that we provide a comprehensive service, while participants are also able to take an active role in determining the care they receive. 

The aim of community-based nursing care is to facilitate NDIS participants being able to remain in their own homes (or in shared NDIS accommodation) while they receive care, so that they can continue to live independently. It is not meant to take the place of other medical care, but to be carried out in conjunction with the work of other medical practitioners so that your health and wellbeing can be supported as effectively as possible.

Community-based nursing care designed to suit your needs

Community-based nursing care can take a variety of forms, and provides support for participants with a range of medical conditions that require professional standards of care.

This can include, but is not limited to:

  • Diabetes management and care
  • Managing and administering medication
  • Continence assessments
  • In-home care after surgery or hospitalisation
  • Tracheostomy management
  • Administering subcutaneous Injections
  • Assistance with eating, swallowing and feeding (including enteral nutrition)
  • Blood pressure and blood sugar monitoring
  • Assistance with stoma, catheter, or other complex bowel or urinary issues
  • Pressure injuries and regular wound dressing
  • Respiratory conditions and ventilation
  • Palliative care

While our support workers and NDIS nurses provide high intensity care for participants, at the same time individual privacy and autonomy is always respected, as the aim of community-based nursing care is to enhance your capacity to live independently as far as possible. 

Additional community-based nursing support

In addition to the medical services described above, as in home care Brisbane specialists our committed team is trained and experienced in providing community-based nursing care for people with a range of complex needs, including being able to provide support for people whose ability to communicate is limited, or who require assistance with behaviour management. 

We can also provide post-acute care and rehabilitation services in your own home, so that you can be discharged from hospital sooner and are able to resume a more independent life as quickly as possible. Our support workers will collaborate wth your GP, OT, physiotherapist and other allied health professionals to devise an appropriate care plan, so that your transition back to your own home is relatively stress free.
