For instance, this can include moving into a new home or shared accommodation, beginning a new job, entering the workforce for the first time, starting a course or going back into education. Whatever the scenario, our dedicated support workers will work in partnership with you to make the transition to a new life stage as seamless and straightforward as possible.

Our support workers assist participants to develop and implement strategies that help to make the transition less daunting, and so that they are better prepared for the stresses that will inevitably occur. This naturally needs to be tailored support, so that it best reflects the needs and circumstances of the individual, and the nature of the transition they are experiencing.

A range of life stage transition supports

The sorts of supports our dedicated and experienced support workers provide depends on the needs of individual participants. For instance, they can include:

  • Helping clients to create and maintain peer support networks
  • Mentoring participants so that they can enhance their decision making skills, etc.
  • Skills development designed to build capacity in a new environment
  • Assistance with finances, e.g., budgets, planning, saving, etc.

Plan Management and NDIS support coordination

For the successful implementation and delivery of a NDIS plan, and to assist in ensuring you have access to the right services, it is important for participants to understand how funding works. NDIS funding will pay for services in three categories: 

  • Core Supports
  • Capacity Building Supports
  • Capital Supports

At Memba Care, our experienced NDIS support coordinators and plan managers can assist and guide your through the process of applying to become an applicant and developing a roster of care submission. This is then submitted to the National Disability Insurance Agency (NDIA), which is ultimately responsible for determining the reasonable and necessary supports which you can fund under the terms of your plan. 

Your support coordinator will then help you to choose services that are most appropriate in terms of making the transition to a new life stage, like those outlined above. In this way, you can be sure that your NDIS funding is being used to help you achieve your development goals, and is enabling you to participate more fully in the wider community.

If necessary, we can also help you to access other sources of funding that may be available in order to support you through a period of transition. This could be through state government funding, the Commonwealth Home Support Programme (CHSP), or other sources.
