Our NDIS support workers specialise in providing in home care Sydney wide, as well as group and centre-based support and community-based support services.
At Memba Care, we pride in home care Sydney NDIS participants can rely on to support them in living as independently as possible. This can include assistance in completing a variety of daily living activities, along with nursing services that support clients’ ongoing health and wellbeing.
Further levels of support include assisting NDIS participants to remain connected to the local community in variety of ways, such as being able to take part is sports and social events, as well as providing assistance to attend doctor and other healthcare appointments, or learning activities that help clients as they work towards achieving individual development goals.
In addition, our in home care Sydney services also include providing support for participants with a range of complex care needs, both at home and in shared NDIS accommodation. Our trained support workers work with doctors and other healthcare providers to develop personalised plans tailored to participants’ specific support needs.
Memba Care also provides a variety of group, centre and community based services that are geared towards supporting participants to take part in social and recreational activities. They provide opportunities to interact and engage with others in an inclusive setting, as well as the chance to develop capacity and work towards fulfilling individual development goals.
We also provide NDIS participants with opportunities to take part in Innovative Community Participation programs, in order to access additional activities beyond regular community supports. These programs are centred around individuals’ interests and help to build capacity and increased social competency.
In addition, we are able to offer community-based nursing care as part of our in home care Sydney services. This includes care for a variety of medical conditions, including diabetes, complex bowel issues, administering subcutaneous Injections and more.
One of the most important services we provide is support clients applying to become NDIS participants. We work with you throughout the application process, and once a plan has been approved, we assist new participants to find appropriate providers and to reach service agreements that deliver value.
Alongside this, we also work closely with participants to delver a range of skills development programs that are designed to enhance capacity and promote independent living. This can include daily living activities such as shopping, cooking and gardening, so that participants are better equipped to remain living at home or in shared NDIS accommodation.
Our in home care Sydney services also support NDIS participants undertaking important life transitions such as moving house, starting a new job, entering education, etc., through skill development and capacity building. In addition, we help clients who are planning to enter the workforce, either through an Australian Disability Enterprise (ADE) organisation or any other type of business.
We assist in the setting of appropriate employment goals and the application process, providing relevant training and coaching as required. Once in work, we also provide support to help participants maintain employment through helping with job customisations as required.
We also assist participants to find appropriate short-term or permanent accommodation, and then to develop capacity that enables them to maintain a tenancy and continue to live independently as far as possible.
