Memba Care is a specialist in home care Gold Coast provider offering NDIS participants daily living supports, as well as community-based services and support coordination.
Our team of caring and compassionate support workers provide NDIS participants with a range of in home care Gold Coast services in their own homes, in shared accommodation, and in the community. Our disability support and in-home care services include assistance for all types of daily living activities (e.g., shopping, gardening, meal preparation, etc.), as well as nursing care that helps to supports participants’ overall health and wellbeing.
In addition, we also provide support for NDIS participants with complex care needs and who require high intensity daily personal activities support. We work in collaboration with GPs, families and other healthcare providers to devise and deliver personalised plans based on individual needs, with the aim of supporting participants to live independently as far as possible.
At Memba Care, we not only provide in home care Gold Coast services, but also a range of group, centre and community based supports for NDIS participants of all ages. This includes community based nursing care delivered in conjunction with GPs and other allied health providers, such as assistance with managing bowel issues or diabetes, as well as support for people recently discharged from hospital.
The aim is always to help participants to live as independently as possible at home or in shared NDIS accommodation. We also provide a variety of group- and centre-based supports that enable NDIS participants to enjoy social and recreational activities in a welcoming, inclusive environment. This helps to promote social engagement and is also a useful way of enhancing overall health and wellbeing.
Support coordination is a valuable part of our in home care Gold Coast services, as it provides assistance for new NDIS participants throughout the application process, and also once a plan has been agreed. We work alongside you to ensure that you fully understand everything to which you are entitled, what budgets you have access to, as well as how to engage providers and reach service agreements.
Alongside of this, we also offer skills development programs that help participants to enhance capacity in a range of different areas, enabling you to achieve your individual development goals and also to participate in the wider community as far as possible. These supports can be on an individual basis, or delivered as group activities.
Additional services that we provide for NDIS participants are centred around employment and accommodation. We work closely with clients to help them gain access to work, both through connecting them with businesses and Australian Disability Enterprise (ADE) organisations, as well as providing assistance in preparing a CV and going through the interview process.
Once in employment, we continue to support clients through job customisations, if required. In addition, as part of our in home care Gold Coast services, we can also provide short-term accomodation in an inclusive, fully supported environment, as well as provide assistance to secure appropriate permanent housing and/or organising shared NDIS accommodation.
