We are in home care Nambucca Heads providers who support NDIS participants of all ages living in their own home or in shared accommodation with a range of services.
Memba Care provides centre-based and in home care Nambucca Heads wide, and we work in close partnership with NDIS participants to deliver a range of services. For participants with Supported Independent Living (SIL) funding (and others), we provide supports that better enable them to live independently either at home or in shared NDIS accommodation.
Our caring and compassionate support workers provide assistance for a range of daily living activities, as well as supporting participants to engage with community life, We can also support participants with complex care needs and provide high intensity care for a range of health and medical conditions.
As well as in home care, Nambucca Heads NDIS participants can also enjoy support in a range of group and community settings. For instance, we provide group and centre-based care services that enable participants to enjoy fun and engaging social and recreational activities.
These inclusive group and individual activities, such as music and arts & crafts, not only enable participants to enhance capability and develop life skills, but they also provide opportunities for socialisation and to develop friendships.
We also offer Innovative Community Participation programs, where participants can take part in social and cultural activities based on their own individual interests, and which would otherwise fall outside the scope of other community-based supports.
As part our packages for centre-based and in home care, Nambucca Heads NDIS participants can take part in skills development programs that are geared towards enhancing capacity and life skills to support independent living. Programs can be delivered in either individual or group settings, with funding able to be met from either a Core Support or Capacity Building Support budget.
Crucially, we can also support participants to develop capacity and skills that will better enable them to understand and manage their NDIS Plan. In this way, participants can feel more confident that they are working with the right provider(s), accessing the appropriate services, and getting full value from their plan.
We work with NDIS participants to provide short-term accommodation in a centre or group residence, accompanied by high levels of personal support in an inclusive, welcoming environment for up to 28 days a year. This support can be paid for out of a STA or Core Supports budget, and is designed either to support participants working towards their individual development goals, and/or to provide respite for family members providing informal care.
In addition, we provide accommodation and tenancy support services where we work with you to find accommodation that meets your specific needs (either short term or on a permanent basis). Following on from this, our support workers can then assist you to further develop life skills and capacity so that you are able successfully to manage and maintain your tenancy.
