Memba Care is committed to providing a wide range of in home care Melbourne services for NDIS participants that are designed to promote health, wellbeing and independent living. This support can take the form of assistance with daily living activities, such as meal preparation, cleaning and gardening, as well as nursing and other healthcare services.
In this way, participants are better able to remain living in their own home (or in shared NDIS accommodation), while at the same time maintaining connections and engagement with the local community. In addition, our caring and compassionate support workers also work with clients who have complex care needs through providing high intensity daily personal activities support.
We work in partnership with family members, GPs and allied health providers to deliver personalised care based on each individual’s needs and the level and frequency of support they require.
Although we are in home care Melbourne specialists, our services also include an extensive range of group, centre and community based supports. For instance, we deliver community-based nursing care in conjunction with other healthcare providers to ensure NDIS participants receive the levels of personalised care required.
This includes the management of health conditions like diabetes and administering subcutaneous Injections, as well as rehabilitation services and care for patients on release from hospital.
We also provide participants with access to a wide range of group and community activities that promote social inclusion and help to develop life skills. Participants can take part in individual and group activities like arts & crafts, games and music, all in an inclusive and supportive environment. These activities are also directed towards assisting clients to work towards their individual development goals, promote wellbeing and reduce feelings of isolation.
We are able to assist people who are applying to become NDIS participants at every stage of the process. Our support coordination services include providing help to make an application and get a NDIS Plan approved, and then supporting new participants to engage appropriate service providers, based on their budget and specific needs.
Our team also works with participants so that they have a full understanding of what they are entitled to under their plan and in order to ensure they are receiving full value from their funding, Memba Care can also deliver skills development programs designed to promote independent living.
These can include programs in both individual and group settings, and which are focused on capacity building and life skills, so that participants feel more confident in a range of settings both inside and outside the home.
As well as centre-based and in home care, Melbourne NDIS participants can engage our support in a variety of other ways. For instance, we provide a range of supports designed to assist participants to enter or return to work. This includes connecting them with potential employers and Australian Disability Enterprise (ADE) organisations, setting goals, and navigating through the job application process.
Once in work, we can also support participants to build capacity so that they are better equipped to maintain their employment. We can also provide short term and holiday accommodation for participants (either through STA funding or from Core Budget) in a group setting with high levels of personal care. This can be a part of an individual’s development goals, or to provide respite for a family member delivering informal care.
